COVID-19 Vaccination Clinic at Chelsea High School

Dear Chelsea High School Community,
On Thursday, January 20, a COVID-19 Vaccination Clinic will be held at Chelsea High School for CHS students and staff. The clinic will be from 9:00am-1:00pm in the Blue Gymnasium. Students will be called out of class to receive their vaccine, and return to class afterwards.
This is an opportunity to receive a free vaccine, regardless of your insurance or immigration status. Initial doses of the vaccine as well as booster shots will be available at this clinic. To receive a booster shot, five months must have passed from when you received your primary COVID-19 vaccine.
Students must complete a registration form for the vaccination clinic. The form will allow parents to provide consent and select an appointment for their student to receive the vaccine. Parents can access the form by using this QR Code, or by clicking here.

We are happy to provide our Chelsea High School students an opportunity to receive their vaccine or booster in school, and we hope you take advantage of this opportunity by registering for Thursday’s clinic as soon as possible. The sooner you register the more flexibility you will have to select an appointment that best fits your schedule.
