• Vaccination Clinic at Chelsea High School

    This Thursday, July 29, a vaccination clinic will be offered at Chelsea High School for students of Chelsea Public Schools and their families. 

    From 12:00-4:00 p.m., visit Chelsea High School to get your first vaccine dose. Students 12 and older as well as their families are eligible to attend. The vaccination clinic will also feature gift cards for those who get vaccinated as well as entry into a raffle to win an iPad. 

    Parents will need to complete the Massachusetts Consent Form for People Under 18 Years of Age as well as the Pre-vaccination Screening Form if you can not attend with your child. Hard copies of the forms will also be available on-site if you are unable to complete them electronically. A registration link is also available for all attendees who wish to get their first dose of the vaccine. All three forms are available via the links below:
